On your Quarterly Payroll Record, Form UI-C54, for the ^[[1st]] quarter of the year 193 ^[[8]], you reported ^[[1]] employees without insurance account numbers.

An employee's insurance account cannot be established without a number.  (This number is the one issued by the Federal ^[[underlined]]Social Security Board.[[/underlined]] )  Will you, therefore, kindly inform these employees to obtain social security account numbers immediately.  Upon application, on Form SS-5, the Federal Social Security Board will issue a number to any employee irrespective of age and/or employment.

Kindly notify this office of the number issued to each of these employees as soon as received.
