Subject: Belgian Citation.

There is enclosed herewith a foreign decoration authorization certificate awarded to you by the Belgian Government. This authorizes you to wear the Officier de l'Ordre de Leopold.

It is requested that you furnish this office with a receipt for the same, using the enclosed blank 
and penalty envelope.
