Dear Miss Lee:

I take pleasure in enclosing a letter received from Mr. G.H. Hughes of Paramount Pictures, Inc., in which he enclosed a new application form for your social security number. Because you signed your previous form as Lee Ya-ching which was not accepted by the Social Security Board, we have to ask you to re-execute this paper by signing your name on Line 16 as Ya-ching Lee. Kindly sign this paper by following his instruction and return to this office for transmission to Mr. Highes.
