Commissioned October 1965 - 1600 RVR day, 1200 RVR night

1. Waiver issued by FS-700 authorizes a gap on the inner 1500 of approach lights.

2. The L-850 centerline lights will be required for 1200 RVR day.

3. As a result of a radio theodolite flight check conducted October 12, 1967, it was determined that a glide slope did not meet Category II criteria. A special maintenance project to restore this facility to Category II capability necessitated shutting down the glide slope on October 30, 1967, for modification. Following rework should subsequent flight check indicate continued unsatisfactory performance, air carrier authorization for Category II operation at this location will be rescinded from individual specifications.

4. Form 511 (Standard Instrument Approach Procedures) expected to be published 12/1/67. Air Carrier authorization approved on individual specifications.

5. Critical area markings required. Sponsor will be requested to accomplish same.

6. Inner marker installed but not commissioned account of a defective antenna. Expected commissioning is now November 30, 1967.

7. Project request being submitted to provide continuous power on Runway 10L in lieu of 28L. Estimated completion, March 31, 1968.

8. Continuous power for owner facilities is estimated to be completed June 1968.
