Gentlemen:

The Federal Highway Administration on behalf of the National Highway Safety Bureau solicits your proposal for a research project entitled "Emergency Medical Care - Demonstration Project."
                                    Plan Demonstration Project
Enclosed are pertinent papers for your use in preparing a proposal.

1. Sample contract with applicable general and special provisions for a cost-reimbursable or firm fixed price contract.
2. Outline of services required.
3. Three (3) copies of Form 1028, Contract Proposal.

Your proposal should detail the elements of work involved, the level and number of personnel to be utilized, and a breakdown of costs, (labor, materials, overhead, etc.). Form PR-1028, Contract Proposal, should be completed in duplicate and returned with your proposal. The Federal Highway Administration estimate for this research project is approximately 90 man-months. You must detail how you intend to accomplish each objective covered herein. The proposals will be evaluated and the prospective contractor selected principally on the following criteria:

A. General Quality and Responsiveness of Proposal

1. Completeness and thoroughness
2. Recognition of overall concept and objective
3. Responsiveness to requirements, terms, conditions and time of performance.
4. Consideration of related factors
5. Physical facilities, if applicable

B. The Qualifications of Staff to be Assigned to the Project
