I am directed by the Commissioner to communicate with you in reference to the recent instructions of the Secretary of War, requiring all Medical Officers, both of the Regular and Voluntier forces on duty with Assistant Commissioners as Surgeons in Chief of Districts to be relieved from duty in the Bureau, and the latter to be mustered out of Service, with the request that you would designate the most suitable person to releive the Medical Officer now on duty at your Hd: Qrs: his duties as Surgeon in Chief of the District. 

So the duties of this position are such as require discretion, foresight, good Judgement and administrative ability, as well as professional attainments such as you could confide in, for managing the Medical Affairs of your District, 

I am instructed to require what rate of pay you would deem advisable or suitable for such a position -
