You are advised that the Department has approved your appointment as a Special Dental Consultant for general field duty with the U. S. Public Health Service at a compensation of $10.00 per diem, when actually employed and travelling expenses, effective February 1, 1930.

The enclosed oath of office forms should be executed before a notary public or other officer authorized to administer oaths for general purposes and returned to this Bureau in duplicate together with standard forms 6.
