In connection with the recent purchase of Robert Indiana's The Demuth American Dream No. 5 bu the Women's Committee of The Art Gallery of Toronto, we would like the artist to fill in the facts required on the enclosed information sheet and return this to us as soon as conveniently possible. Since we do not have Mr. Indiana's address I hope you will not mind seeing that this form reaches him. We are always anxious to get the artist to fill in these facts whenever it is possible, so that our accession records may be as complete as possible. We will appreciate it if you can attend to this matter for us.

We look forward to receiving this painting in the near future once the exhibition in The Tate Gallery is over.
