I want to thank you on behalf of the Contemporary Art Society for your friendly cooperation in lending the Robert Indiana painting, 666, to us for our January exhibition, "Painting and Sculpture Today."

I am enclosing a loan form in duplicate (you may keep the blue sheet), which I will appreciate your filling out and returning to me at your earliest convenience. Will you kindly indicate on the form the artist's birth date as well as the sales price and insurance value. 

I will be in touch with you later regarding shipment. As it appears now we will have Manhattan Storage and Warehouse Company pick up and deliver the painting to Indianapolis.
