Louisville, Ky., Nov. 12th, 1866.
  Respectfully returned to S.F. Purcell, Glasgow, Ky., who will make his accounts for services rendered and medicines furnished to U. Troops, upon forming similar to the enclosed blank.
 The account set forth-the name of each person assisted and the date, as, also, the separate charge for medicine, and must be accompanied by his own affidavit, that the account is correct, and just; that the charges are customary charges of the place, and that he has never before received payment for the amount claimed, and also be accompanied by the certificate of the officer under them.  No services are rendered to the effect that they are correct, and stating the circumstances which renders his employment necessary.  The account must also be accompanied by the amnesty oath.
