Dr. Cooper will state the nature of the disease or wound in each case, and make affidavit instead of certificate as to correctness, &c. of the account, and will incorporate with the affidavit

Part of this account is for services rendered prior to the decision of the Secretary of War directing accounts of this nature to be paid from the funds of the Bureau.
