I have the honor to apply for a leave of absence of twenty days, with permission to apply for an extension of ten days, upon a Surgeon's Certificate of Disability herewith enclosed.

I would respectfully state that owing to the increased duties lately devolving upon me, I have suffered from ill health and old wounds, so much so, that I am unable to perform the duties required of me without some rest and medical attendance. 

I would also state, that the interests on business of this Bureau will not suffer by my absence, as I have made an arrangement with a friend, a gentleman from New York City, to take charge of the Office, attend to Freedmen's Complaints &c &c during my absence.
