Sir,
In place of the usual form of Monthly Report of Station, Duty &c. for the month of August, I have the honor to report as follows: That after having been more or less ill for some time with fever and ague, diarrhoea and dysentery, I was on the 15th of August completely prostrated and laid up thereby. That I thereupon made application to Surgeon J.J. De Lamater for Leave of Absence, which application was returned to be forwarded through proper channels. This was duly done, but I did not wait for the return of the same, being too ill and utterly unfit for duty.

This Leave of Absence if granted, has not yet reached me, being sent, I suppose, to Fort Monroe. Hence I cannot fill out the usual form of Report of Station, Duty &c. My application was
