Office Superintendent R. F. & A. L., 1st Dis't of Va.,
Norfolk, Va., March 18th, 1867

PERSONALLY appeared before me, a Commissioned Officer and Superintendent of the Bureau of R. F. & A. L., William Artis who being duly sworn, declare that he is the identical William Artis who was a Private in Company "H" of the 38th Reg't of U.S. Colonel Volunteers, and who enlisted in the service of the United States, at Norfolk Va, on the 4th day of August 1864, and was discharged from the service of the United States at Indianola Texas on the 25th day of January 1867. That he claims 100 dollars bounty retained under General Order, No. 90. Head Quarters, Department of Virginia and North Carolina, Series, 1864. [[strikethrough]] That he never received a certificate of retained bounty, or that he lost his [[/strikethrough]] certificate of retained bounty enclosed herewith [[strikethrough]] on or about the   day of 186  [/strikethrough]]
That I William Artis have made, constituted and appointed, and by these presents do make, constitute and appoint Bvt Brig Genl O. Brown US Vols [[strikethrough]] Capt. JAMES A. BATES, Act. Asst. Adgt. Genl., my true and lawful Attorney, for me and my name, place and stead, to draw and obtain for me the sum of 100 dollars and -- cents, now deposited in and with the Bureau of R.F. & A.L., Washington, D.C., and also to deposite the same with the Freedmen's Savings and Trust Company at Norfolk Va in my name immediately upon its receipt by him.

ALSO, Personally appeared before me, William Mullen ad George Skinner of the County of Norfolk and State of Virginia who being duly sworn, declare that the above named applicant was a Private in Company H of the Thirty eighth Regiment of U.S. Colored Volunteers, and know him o be the identical person named in the foregoing declaration; and that they have no interest whatever in the application.
