Sir,

I have the honor to request in behalf of George Martin, late Priv: Co. B, 39th U.S. Inf, discharged at Ship Island 29th Oct. 1868, on surgeon's certificate of disability, that you will furnish him with a certificate, signed in your official capacity and dated, containing a statement of all the facts known to you concerning the kick from a mule received by him, time; place; manner and all the circumstances under which the inquiry occurred, the duty, service or situation of the soldier at the time the injury was received, and whatever facts may aid the judgement as to the case, immediate or remote, of the disability and the circumstances attending it.
