Samuel Moore Freedman presents himself at the Office of the Bureau Refugees, Freedmen &c at Lauderdale Miss on this the 29th day of December, and makes oath to the following viz.

1st I am blind and have been helplessly so for the last fourteen (14) months - I belonged to the 2nd Light Artillery — U.S. Colored rendezvous at Memphis, Tenn. I had sore eyes while in the service. — Have never received any bounty, have lost my discharge papers.

Further being totally helpless, I am unable to get witnesses to prove my asertions &c. I will say that I was under Capt Lewis B. Smith, Officer Commanding Major [[Rombow?]]. Being in a condition as above stated, I pray that I may be allowed all the benefits according to the laws provided in such cases — viz. Pensions &c.
