I Mrs Nancy Park declare upon oath that 
I have no ablebodied Father 
Mother, Husband, or Relative that can 
furnish subsistence for me, no provisions 
except Govt, no money, real estate 
houses boughten or other property that 
can be converted into money.  
That I have two children depending 
on me for support and that 
I have made every effort to obtain 
employment without success and I 
further swear that I will suffer 
unless furnished with provisions 
by the United States Government
