Transcription Notes:
The month on the top of this form is a bit of a mystery, as James Morris uses shorthand several times in the form. Based on the window of time named in the semester (September 6 to December 20, 1869) I assume the shorthand was November.
The name of the school was also hard to discern. I assume the last work was Crossing and used the shorthand "Xing" but that could be worth looking up or cross referencing?
