Sir:
I have to inform you that Form 41 of this office, of which a supply has already been furnished to you, is designed for the use of the Disbursing Officers in recording the answers of claimants to such questions as may be propounded by them with the view to determining as to their identity; the same to be filed as "office memoranda". It is believed that they will greatly facilitate the requisite examination of claimants as well as afford a convenient record for any future reference that may become necessary.
