NARA 0290 (FORM 24.) Stephen Phillips Private, "E" Co., 109th Reg't U.S.C Troops, was born in Jefferson, Kentucky, was enlisted the 3d day of June, 1864, at Owensboro K'y, by Capt. Grissom, and was mustered in the 3rd day of June, 1864, at Owensboro K'y, by Capt. Grissom, for 3 years. At the time of enlistment he was 29 years old and 5 feet 9 inches high, Black complexion, Black eyes, Black hair and by occupation a Farmer. He was discharged [[strikethrough]] (or died) [[/strikethrough]] 6th day of February, 1866 at Port Lavaca Texas 1st Lieut. Charles E. Hart was commanding officer, and Harry Howard first sergeant at time of soldier's Discharge. [[strikethrough]] The following are the names of one or more of his company who enlisted at or about the same time and place: enlisted day of 186_, and enlisted day of 1866 [[/strikethrough]] REMARKS. 1. If a slave, that fact and name of owner will be noted here. 2. Any other peculiar fact or event connected with the soldier's history will also be noted. Joined at Original Organization on new muster out Roll of Co. dated Port Lavaca Texas, February 6" 1866, he is reported - "Sick in Hospital at Old Point Comfort Va, Since Feby 9"/65. No discharge furnished." He was furnished with a discharge March 9" 1866 at Louisville Ky, by Capt Snyder as of date of Muster out of Organ'n. "Slave" of Thomas L.
