DISCHARGED SOLDIER.

State of Tennessee 
County of Maury } ss.

On this Third day of June, 1878, personally appeared before me, The County Court Clerk in and for the county and State aforesaid, John Miller, who, being duly sworn according to law, declares that his age is 34 years; that he is a resident of Santa Fee in the county of Maury, State of Tennessee, and that he is the identical person who was a Private in Company "A" of the 15" Regiment United States Colored Troops; that this declaration is made for the purpose of securing payment from the Disbursing Officer of the Freedmen's Branch, Adjutant General's Office, of the bounty, &c., to which he is entitled, as per settlement of his claim made by the Accounting Officers of the Treasury. That his post office address is Santa Fee, Maury Co Tennessee

Also personally appeared before me Plummer Webster and Pitts Jones, of the county of Maury State of Tennessee, who, being duly sworn according to law, declare that for 20. years and 25 years, respectively, they have been acquainted with John Miller, the above-named applicant, who was a private in Company "A" of the 15" Regiment of United States Colored Troops, and know him to be the identical person named in the foregoing declaration; and that they have no interest whatever in the claim referred to.

This affidavit should not be accepted unless executed by two persons who have known the claimant sufficiently long to render them COMPETENT identifying witnesses.  No witness can be competent to testify as to the SERVICE OF THE SOLDIER unless the acquaintance covers a period from twelve to fifteen years.

NOTE. - In all cases where a mark is substituted for the written signature, two disinterested witnesses are required.
* When the claimant lives in a city, the street and number should be given.
