Very difficult to distinguish location I admit to being uncertain in this case as to the collector number but it does appear that the collector date is 1888. Certainly other collections by this collector have been recorded around this time. Calkins. Dr Uylanden appears to be the person who "descr & publ." ie described and published it. So I've edited the collector box using the dropdown menu.
