NARA 693 (Form 24.) Humphrey Waller, Private Co., "C" 13th Regt. U.S.C. Art'y (H.) was born in No record, State of Kentucky, was enlisted the 4th day of February, 1865, at Paducah K'y, by Captn. H.H. Morgan, and was mustered in the 27th day of February, 1865, at Paducah, K'y., by Captn. Jno H. Morgan Pro: Mar, for 3 years. At the time of enlistment he was 19 years old, and 5 feet 9½ inches in height. He [[strikethrough]] was discharged (or [[/strikethrough]] died) 19th day of March, 1865, at Smithland K'y, in Reg'l Hospital; Captn. William Botimer was commanding officer, and Charles Mason First Sergeant at time of soldiers Death. The following are the names of one or more of his company who enlisted at or about same time and place: Dunk Wathen enlisted 4th day of February, 1865, and William Williams enlisted 4th day of February, 1865. REMARKS. 1. If a slave, that fact and name of owner will be noted here. 2. Any other peculiar fact or event connected with the soldier's history will also be noted. Joined Co. at Original Organization. (Cause of death - Pneumonia.) (It appears from Records of Enlistment & Musters on file in this Office, that he was a "Slave" of Nathan Waller.) (Occupation Laborer. Eyes, Black. Hair, Black. Complexion, Yellow.)
