I was ordered out on Gard duty & therefore failed to be Mustered into Service & was also Granted a furlow to visit my afflicted Family, & consequently failed to be mustered out of service, but notwithstanding the facts herein stated as afforesaid stated, are true and can be verified by the Officers of the Company & Regiments.

You Will please reply at Your earliest convenience & hope send me the necessary Blanks.
