Women's National Air Meet,

Dayton, Ohio:

1. I, the undersigned, hereby make application for entry in the First Annual Women's National Air Meet to be held at the Dayton Municipal Airport, Vandalia, Ohio,, August 4 and 5, 1934.

2. This application is subject to the following rules, all of which I hereby accept. It shall not become an entry until it is accepted by the Contest Committee.

3. I hereby agree that I will familiarize myself with and abide by the rules and regulations of the 1934 Women's National Air Meet, as prepared by the Contest Committee, and that they shall be considered as a part hereof; also that all matters not covered in such rules shall be subject to the rules and regulations of the F.A.I.

4. I hereby assume and accept full responsibility and hereby agree to indemnify and save harmless the Women's National Air Meet, the Dayton Municipal Airport, the City of Dayton, all Managers and Personnel, from any and all damage to persons or property caused by myself, my agents or representatives, arising from the operation, storage or maintenance of my airplane or equipment during the continuation of the Women's National Air Meet for 1934 and during any period that I, or my agents, servants or representatives shall be using the fields of the race course on which the Women's National Air Meet shall be held.

In view of the fact that certain of the races are to be handicap events and that my airplane will be tested by some competent pilot, properly selected, I furthermore agree to accept full responsibility for my airplane during such tests and to relieve all concerned, including the management of the Air Meet, the management of the Municipal Airport, the City of Dayton, the test pilot and the company employing him from all claims for damage or liability during the testing of my ship or after the result of such testing.

5. It is further agreed that any announcements, notices or decisions sent to me or to my representatives, shall be deemed properly addressed if directed in accordance with the address given under my signature.

6. If there are less than five ships on the starting line for any event, the contest can, at the option of the Contest Committee, be declared no race, and the prize money for this event will not be distributed.

7. There will be an entry fee of five ($5.00) dollars, which covers all the events, entry fee of $5.00 to be refunded provided the entrant is on the starting line ready to start at the specified time and starts in all the events in which she is entered. 8. Entries will close at midnight Thursday, August 2, 1934; deadline for late entrance 1:00 P. M. the day of the entered event.

9. Any plane is eligible holding Department of Commerce approved license.

10. Entrance requirements for pilots: F.A.I. certificate and annual sporting license, Department of Commerce private license or higher grade.
