I certify that I have personally examined A. R. Brooks of Cambridge, Mass M.I.T Dormitory as directed by the Commanding General, Eastern Department. 

I find his educational qualifications to be as follows: as stated in application. 

I find his military experience to be as follows: as stated in application. 

I find his physical condition to be as follows: Good. 

I find that he is a citizen of the United States, 21 1/2 years of age, 67 inches in height and weighs about 140 pounds - as stated in physical examination blank. 

I further certify that I would recommend him for appointment in the Officers' Reserve Corps if I were examining him under paragraph 57, Officers' Reserve Corps Regulations. 

I recommend that he be admitted to the Training Corps to be held at Plattsburg Barracks, N. Y.

I acknowledge that I have enlisted for the full period of not exceeding three months for instruction at the United States Military Training Camp to be held at Plattsburg Barracks, N.Y. commencing May 8, 1917, and agree to obey the rules and regulation for the governing of said camps; and I do solemnly swear (or affirm) that I will bear truth, faith, and allegiance to the United States of America; and that during the period of said camp, unless sooner discharged by competent authority, will obey the orders of the President of the United States and the orders of the officers appointed over me, according to the rules and regulations established by the Secretary of War, and to accept such appointment in the Officers Reserve Corps as may be tendered to me by the Secretary of War.
