DEPARTMENT OF ANATOMY
HARVARD MEDICAL SCHOOL
BOSTON, MASSACHUSETTS

June 5 - 1917

Mr. Arthur R. Brooks
534 State St. Bridgeport Conn.

Dear Sir-

Your letter at hand and you may present yourself for physical examination for aviation service any week day at 9 a.m. at Harvard Medical School Boston (Room 322, Bldg B)

These examinations are special for aviation, and made only by specially authorized boards.
I know of none in the vicinity of Bridgeport. There is one in New York, however.

I have your application and testimonials here, and you could get your examination elsewhere only by authority of the Chief Signal Officer Wash, D.C. who might, on your applying to him, have the papers in your case transferred to the New York board.

Respectfully
Geo. H. Crabtree
Lt. Col. U.S.A. retired
