C. INSURANCE CONDITIONS - CLAIMS ARISING FROM AN AIRCRAFT ACCIDENT

1. The Company insurance policy provides that prompt notice is given of every accident or claim, that the Company promptly deliver to the Insurance Company every communication, process, pleading and paper relating to any claim or accident; that the Company does not make any admission of liability; that the Company does not incur any expense, settle any claim or interfere in negotiations for settlement or in any legal proceedings except only that the Company may provide, at the time of an accident, such immediate relief as is imperative.

2. If claim or inquiry regarding a claim is made in person, obtain:

(a) Injured person's name 
Home address - Phone number
Business address - Phone number

(b) Flight number, station of origin, station of destination, exact location of occurence [[occurrence]] of injury, date and time of occurence [[occurrence]].

(c) Complete description of occurence [[occurrence]] of injury, nature of injury, where, when, and by whom created; and

Instruct claimant to write letter, giving all details to the Treasurer.

3. If claim is made by letter, make a simple acknowledgement stating that the matter is being referred to the Treasurer's office for attention. Forward original letter, with copy of acknowledgement, to the Treasurer.

D. LEGAL ACTIONS ARISING FROM AN ACCIDENT

Whenever any legal action against the Company or involving an employee comes to the attention of an employee, he shall immediately so notify the Treasurer by telephone. Failure to notify him promptly may result in a judgment being rendered against the Company by default.

E. In the event of employee fatality all matters pertaining to settlement of the employee's account with the Company, including salary, expense advances, vacation accruals, insurance, etc., will be summarized by his department head and submitted to the Vice President (J. E. Gallagher) for final settlement.
