1.8 APPENDIX A

AGREEMENT TO BE SIGNED BY INJURED PASSENGERS TO COVER MEDICAL AND HOSPITAL EXPENSES

(Passenger's Name), hereby request and authorize New York Airways, Inc., to advance for my account, until further notice, such funds as may be needed to pay medical and hospital expenses arising out of the treatment of injuries suffered by me in the accident which occurred at (Place of accident) on the [[blank line]] day of [[blank line]], 19[[blank line]], with the distinct understanding that this agreement does NOT constitute a release of claims, demands or rights which I may have against New York Airways, Inc., as a result of said accident, and that the advance of funds by New York Airways, Inc., in accordance with this authorization does not constitute an admission of liability by New York Airways, Inc., for damages arising out of said accident. It is further understood and agreed that such advances are repayable by me and that any unpaid balance of such advances shall be deducted from any sum which may become due to me from New York Airways, Inc., as a result of such injuries.

APPENDIX B

IDENTIFICATION INSTRUCTIONS

Suggestions for Identification in case of Fatal Accident:

A. If the helicopter is a total wreck and death to all occupants has been instantaneous, there arises the problem of identification of the bodies. This work of identification is greatly appreciated by relatives; the Company wants these relatives to have what comfort can be given them by making a painstaking study of identification.

B. As the bodies are taken from the wreck, it is important to remove with each body, insofar as possible, any apparel or personal effects, which belong to that particular body. As this is done, the physician should dictate or write down complete descriptions of each body examined. The body should be tagged immediately and a duplicate tag
