Capt. Jay N Skelton of Co "K" 58 Regt Ill Vols in charge of "Home Colony" near Montgomery, Ala., having applied for a certificate on which to ground an application for leave of absence. I do hereby certify that I have carefully examined this Officer, and find that he has been suffering from chronic Diarrhea and Remittent fever for the last (2) two months. I further declare my belief that a leave of absence is necessary in order to save life and that he will not be able to resume his duties in a less period than (30) thirty days.
