Sir:

I have the honor to inform you that my health being sufficiently reestablished for the performance of duties, I would respectfully request that information may be furnished me. 

As respects the case at the Bureau Orphan Children "which requires amputation" - represented in you by Capt. A. Rutherford if I undertake the charge of this case, will I be permitted to carry it through before the annullment of my contract?

Should a private physician be employed instead, the fees, as established by the Medical Society of this County, would [[serve?]] the sum for attendance, in an amount exceeding three hundred dollars.
