INSTRUCTIONS

1. Application and all attachments will be submitted in duplicate
2. Give a complete list of all attachments.
3. In Item 3, if you are at present a member of the Regular Army, Organized Reserves, Federally recognized in the National Guard, state which, giving grade and organization or granch of service.
4. In Item 4, give names of aviation schools attended with dates of graduation also a brief resume of your flying experience.
5. In Item 5, give nature of your present aviation activities, (e.g. employed in commercial or governmental aviation, a member of a flying club, student flyer, or an owner of an airplane for business or pleasure). State name of aviation organization you are connected with and in what capacity.
6. In Item 8, attach copies of orders of rating.
7. In Item 9, attach copies of form 152, AGO.
8. In Item 10, attach Exhibit "C".

     The following is a brief outline of the requirements prescribed for the military rating of AIRPLANE PILOT, in the Air Corps Reserves.

     The flying time requirements for the above rating are that the applicant shall have flown as pilot of heavier-than-air craft not less than 200 hours of which not less than 75 hours were alone, 100 hours were in service or equivalent type of airplanes and not less than 20 hours were as pilot within the 12 months preceding date of applications. Only flying time certified to by authorized persons other than the applicant (e.g. Operations Officers, Department of Commerce officials, authorized agents of commercial aviation companies or operating managers of flying fields), will be accepted as authentic. The provisions of 100 hours in service or equivalent types of airplane may be waived in case the applicant has flown 400 hours or more in heavier-than-air craft while acting as pilot. Airplanes powered by 300 horse power or over are considered to be the equivalent of service type airplane. The provisions of 75 hours alone is defined as being pilot of an aircraft in which there is no other qualified heaver-than-air pilot. Applicants will be required to pass a flying examination on service and training type airplanes.
     Applicants will be required to pass a written examination in the following subjects: Theory of flight, Aerial Navigation; Meteorology; Airplane engines and Laws and Regulations pertaining to flying. Applicants who at any time hold a military rating as pilot of heavier-than-air craft may be exempt from the entire written examination except in Laws and Regulations pertaining to flying. Exemption will be granted in any subject in which the applicant holds a certificate of completion of an Air Corps Correspondence Course (Form 152, AGO).
     Only applicants who have the flying time requirements and who  have been found physically qualified for piloting duty, will be permitted to take the written and flying examination.
