(b) Properly authenticated transcript of educational 
qualifications.¹
(c) Fingerprint records.
(d) Three or more letters of recommendations, and at least three references of individuals who are prominent in the community, who may be personally contacted for 
information regarding applicant. These letters and  references should serve to furnish information as 
to the applicant's moral, mental, and professional 
qualifications.¹
(e) Report of physical examination.
(f) Two recent photographs of the applicant's head and 
shoulders, one profile, and one full face, each 
about 2½ by 2½ inches.
(g) Statement as to prior military or naval service.
(h) Statement as to receipt or nonreceipt of pension, 
compensation or retired pay from the Government of 
the United States, or that application for such is 
or is not pending.
(i) Statement from employer or responsible authority as to availability for active duty in mobilization.¹
(j) Report of interview by commandant or officers 
designated by commandant to interview or investigate 
the candidate. this report should afford reviewing
officers a clear picture of the applicant's 
prospective value as a Naval Reserve officer, with 
special reference to officerlike qualities as distinguished from technical or professional qualifications, which latter are otherwise reported upon.
(k) Statement from applicant for appointment in Supply 
Corps that he will, if appointed, file partially 
executed bond, signature cards, and application for 
bond.¹
(l) Agreement to enroll in correspondence course.¹

AGREEMENT TO ENROLL IN CORRESPONDENCE COURSE

All applicants for appointment as Naval Reserve officers who execute Form N. Nav. 353 or N. Nav. 392 shall, except as indicated below, execute an "Agreement to enroll in a correspondence course." The following are not required to execute the above agreement:

(a) Fleet Reserve. 
(b) Ex-officers Regular Navy or United States Marine Corps.
(c) Merchant Marine Reserve.
(d) Aviation cadets and officers of class A-V 
(N).
(e) Graduates of United States Naval Academy.
(f) Graduates of naval Reserve Officers' Training Corps units.
(g) Ensigns, Volunteer Reserve (probationary).
(h) Staff officers under agreement to enroll in appropriate correspondence course for their corps.

Other officers, especially Merchant Marine Reserve officers, who are not required to execute an agreement to take the course in Navy Regulations and Customs, are encouraged to do so.

Failure to carry out agreement executed prior to appointment in the Naval Reserve in regard to enrollment in and completion of correspondence courses within the time limit specified, or within any extension of such time limit as may be granted by the Bureau of Navigation, will be considered as sufficient grounds for recommending an officer's discharge from the Naval Reserve.

Upon receipt of the executed oath and acceptance of office of an officer who has agreed to enroll in the correspondence course in Navy Regulations and Customs, the commandant will forward a copy of the agreement to the appropriate Naval Reserve educational center. Upon receipt of such copy the educational center shall enroll him in the course in accordance with existing instructions.

1. If appointed in the united States naval Reserve, I agree to enroll promptly in the correspondence course in Navy Regulations and Customs, and to complete such course within 2 years, and thereafter to enroll in such other correspondence courses as will better fit me for the performance of my duties as an officer in the United States Naval Reserve. I will also participate in such exercises and other activities as may be prescribed for the classification to which I may be assigned, or for naval Reserve officers in general, in the locality in which I reside.

2. I hereby apply for enrollment in the course in Navy Regulations and Customs effective upon execution of oath and acceptance of office as an officer in the United States Naval Reserve.
