A Member under suspension and whose ultimate status is undecided is still a Member. If there are reasons for leaving other than those given above they may be inserted in the proper space. The Financial Record will not always give a detailed classification of the reason for termination of membership. For example, where the Member is merely recorded as having died, it will be necessary to look in the Member's Personnel Folder, to be examined as indicated below, to ascertain the cause of death. It is believed that the Personnel Folder will show the cause of death in all cases. If the death is shown during a period of military service the death can be called a military death, even though the exact circumstances of death are not given. Where the Member is reported as having been presumably killed in action but without an exact date, leave the date of termination blank; put "killed in action" or "missing in action", as the cause of death; on the reverse side of the card give such information, including if possible the Armed Forces Serial Number, as is contained in the Personnel Folder. 

Members Failing to Return Questionnaires

If the cut-off date for the questionnaire returns is October 15, all the cards in the questionnaire file which do not show that an acceptable questionnaire has been sent in should be removed from the file. If it is practicable to run these cards through the mimeograph for over-printing on the form on which the necessary information is to be inserted, that can be done. If it is not practicable to utilize the cards in this way, the same cards as for former Members can be used. In the latter case, the name and A.L.P.A number on the cards not showing a valid return should be entered on the cards not showing a valid return should be entered on the card used for the records of former Members; in the latter case, the questionnaire cards may then be returned to the file in numerical order. In any case, the cards on which the information is entered should be alphabetized.

The Financial Record will not always give a specific classification of the reason for leaving A.L.P.A. For example, when a Member is merely recorded as having died, it will be necessary to look in the Member's Personnel Folder to ascertain the exact cause of death. The instructions for examination of the Personnel Folder are given below in Para. 7. If there is doubt about a cause of a Member's becoming inactive or leaving A.L.P.A., leave the space blank or enter a code in pencil until the Personnel Folder is examined. 

Having entered the three items from the Financial Records, consult the Personnel Folder, and, if the person involved is a Member at present, the Membership card file as well. 

For former Members, the Membership card will have been removed from the Member card files and placed in the Personnel Folder. From this Membership card insert:
