A. The following additional information would be required:

1. Date of birth of wife.

2. Date of retirement of pilot.

3. Total credited compensation of pilot under Air Line Pilots' Retirement System:
(a) for retirement purposes and,
(b) for survivor purposes.

4. Months of service under Pilots' System.

5. Date of death of pilot.

6. Date of death of wife.

7. Number and ages of eligible children

8. Date of widow's remarriage.
