Instructions for Filling in Form

(READ CAREFULLY)

This form was designed to show your stability on your bid trip, how much on duty time you put in and how much time you were away from home, as well as your daily flight time. ALL ENTRIES OF TIME SHOULD BE ON CALENDAR DAY BASIS. Thus if you are in flight or at a layover station you will have to break the time at night and enter the portion before midnight on the previous day and the time after midnight on the next day.

(1) In column one enter your bid trip number on the days of your normal sequence to fly it and the type of equipment to be flown. COPILOTS OR OTHER PILOTS WHO DO NOT HAVE TRIP SELECTION PRIVILEGES WILL LEAVE COLUMN (1) BLANK.

(2) In column two enter the trip numbers you actually flew on the days you flew them, as well as the equipment you flew.

(3) In column three enter the time you report for duty (1 hour before scheduled departure).

(4) In column four enter the time you go off duty. At your hotel layover station or final home trip the time should be 30 minutes after ramp arrival to allow for necessary trip to operations and debriefing. Unless you have eight hours layover at an airport with sleeping facilities or 10 hours at a hotel you are considered on duty. Thus a CHI-LGA trip leaving CHI at 8:00 a.m. arriving 12:30 p.m., leaving LGA at 2:00 p.m. and arriving CHI at 4:35 p.m. would be classified as on duty from 7:00 a.m. to 5:05 p.m. or 10 hours and 5 min. Deadhead time should be classed as on duty time.

(5) In column five enter your total duty time for the calendar day. If you were still on duty at midnight, your duty time would be from the time you reported for work up to midnight; the balance would, of course, go on the following day. If you were on standby reserve count one hour on duty for two hours on reserve and indicate (R) for reserve.

(6) In column six enter the total time you were away from home. This is to be measured from the time you reported for duty at your home base as shown in column 3 to 30 minutes after ramp arrival back at your home base. Here again be sure to break the time properly at midnight.

(7) In column seven enter your flight time for the calendar day so that time prior to midnight is credited in the proper place.

Total your time on duty, your time away from home, and your flight time for the month.

At the bottom of the form insert the number of complete calendar days you were free from all duty with the air line during the month. In other words, report only complete calendar days, defined as, "midnight to midnight."
