LOAN AGREEMENT FOR SPECIAL EXHIBITION

This Loan Agreement has two parts:
1) The condition of the Special Exhibition loan.
2) The catalogue information on a separate sheet for each object included in the loan.

Please complete and sign the two copies of both parts of this form, return both copies of the Loan Agreement and one copy of each Catalogue Information sheet to the Registrar, Museum of Fine Arts. Both copies of the Loan Agreement will ne signed by an authorized agent of the Museum of Fine Arts and one copy returned to the lender.
