In compliance with Extract No. 15. Special Orders. No. 623. War Department, Adjutant Generals Office, November 30th 1865, I have the honor to report for duty in the 
Freedmens Bureau. 
I most respectfully request a "Leave of Absence" for twenty (20) days, (or such time as can be granted,) to visit my home at Ovid N.Y. to attend to important private business.
My present Address is "Washington D.C."
