If a "Stop Payment" request has been made out, DO NOT CASH THE CHECK until you have singed the above release. Failure to follow these instructions may cause the Federal Reserve Bank to refuse to honor the check when presented for payment and will result in embarrassment to you as well as to the person who has cashed same for you. 

If a check is lost or stolen after delivery to the employee, the loss should be reported immediately, giving full particulars. In this case, a "Stop Payment" request should be filled out at once.

Employees will in no case contact the Payroll Preparation Unit or the Disbursing Office direct.
