SUBJECT: REPORTING ACCIDENTS

In order to protect your compensation rights it is imperative that the following rules be observed:

(1)Report all injuries to your Supervisor or foreman immediately. The Supervisor, in turn, must notify this office immediately.

(2)Secure first aid treatment at once for every injury, no matter how slight.

(3)Where more than first aid treatment is required, competent medical and surgical treatment will be rendered injured employees without cost to workers for injuries sustained on projects. It must be also understood that Security Wage Workers are covered by compensation while traveling to work or returning from work.
(4) Failure to report injuries promptly and to make claim may result in loss of compensation.
