SUBJECT: UNEMPLOYMENT COMPENSATION BENEFITS
SPECIAL BULLETIN NO. 67
DATE: MARCH 16, 1938

1. All Security Wage and Project Supervisory Employees with relief status are required to fill in the attached form, "Report on Private Employment." Completed forms must be forwarded to the Division of Employment, 95 Madison Avenue, not later than April 1, 1938. 

2. All private employment during 1937-1938 of the employee and of each member of the family group will be listed on this form. 

3. Employees who were not engaged in private employment during this period will note on the form "active on WPA rolls," "active on relief rolls," or "unemployed," as the case may be.

4. Project Personnel Officers will see that a supply of these forms is placed in the hands of Unit Supervisors and Project Supervisors for distribution to employees, and will be responsible for having the forms filled in and returned by all employees with relief status on projects under their jurisdiction.

5. All Security Wage and Project Supervisor Employees with relief status, eligible for unemployment compensation benefits, are expected to file claim for these benefits. Claims are to be filed in person at the New York State Employment Service.
