I have the honor to request that I may be furnished with the following Educational Blanks 20. form No. 3. 6 form No. 2.

In my letter of the 6th inst. transmitting School Report for November, I requested that these blanks might be forwarded at as early a date as practicable so that they could be distributed to Teachers before the 25th, but as yet they have not been received. As the schools nearly all have vacation until after New Years I fear this will cause great delay in getting in the Reports.
