FORM OF A MEDICAL CERTIFICATE

Lieut Alfred F. Manning of the ___________ Regiment of Veterans Reserve Corps, having applied for a certificate on which to ground an application for leave of absence, I do hereby certify that I have carefully examined this officer, and find that he is wounded by Pistol shot through the right thigh and the right leg both wounds rendering him unfit for duty  Lieut Manning has lost the left leg in service and I think would receive benefit from going to his friends. A change of air would also be beneficial to him
And that, in consequence thereof, he is, in my opinion, unfit for duty. I further declare my belief that he will not be able to resume his duties in a less period than thirty days.
