Captain:
I have the honor to make application for a Leave of Absence for five (5) days for the purpose of visiting the City of New York for the following reason.
 I have this day received official information from the War Department of having been appointed Captain in the Thirty Regiment, Infantry with instructions to report on on before the 24th  and for examination to the Board in session in New York City, and of which Brevet Major General G.G. August, Colonel 12th W.S. infantry is President.
