Bv't Capt. D.J. Connolly of the Veteran Reserve Corps having applied for a Certificate on which to ground an application for leave of absence, I do hereby certify that I have carefully examined this Officer and finds that he is affected with "Chronic Bronchitis" which to the best of my knowledge and belief was contracted by him in the performance of his duties while in charge of the Counties of Pr. Edwards and Cumberland Va during the mos. of February and March, 1866.  He is also suffering from the extreme prostration and debility induced by the last mentioned disease and from a wound received in the performance of his duties at the Battle of Gettysburg.  The wound is still open and suppurating with frequent discharges of spiculae of bone.  And that in consequence there of, he is in my opinion, unfit for duty.  I further declare my belief that he will not be able to resume his duties in a less period than thirty (30) days.
