I have the honor to request that I be furnished with fifty blanks No 4. - Monthly reports on Education.

These blanks were furnished the A.S.A.Comrs for the month of January last direct from your office, and from this, and the fact of my not being supplied with these blanks for distribution led me to suppose that it was the intention to furnish them direct from your office. Two of the A.S.A. Commrs (new appointees) having failed to make a report leads me to believe that no blanks have been sent them - hence this application.
