Dear Mr. Rausch:

Re: Policy No. AZ559705

As requested on the telephone, I wish to give you the details of the accident suffered by Mr. Germain Seligman's temporary maid.

Mrs. Mary Moore

New York City

As related by Mrs. Moore, she fell from a small step ladder "kitchen steps" while at work in the apartment thereby wrenching her back and spraining her right wrist, necessitating medical attention. All told, she disbursed for massages and medications $15.00 which we reimbursed to her.

Mrs. Moore remained absent from her job since the accident which happened on October 20th last, 1945 I was only able to see her on Oct.23 when I obtained the above data. Please also note that she is no longer in the employ of Mr. Seligman.

I should appreciate it if you would take the necessary steps to have Mr. Seligman reimbursed for the above mentioned sum.
