I, Henry Smith of the City of Baltimore, [[strikethrough]] of [[strikethrough]] in the County of Baltimore β€” State of Maryland, do solemnly swear, in presence of Almighty God, that I will henceforth faithfully support, protect, and defend the Constitution of the United States, and the union of the States thereunder; and that I will, in like manner, abide by and faithfully support all laws and proclamations which have been made during the existing rebellion with reference to the emancipation of slaves: So help me God.

It appears from a Certificate of Major A. G Brady Provost Marshal at Point [[S?]] that Henry Smith above named then on 7th June 1865 a Prisoner of War was released upon taking the oath of allegiance to the United States which I have advised him now to renew it [[adding?]] to the above form.β€”
