Charles B. Burnett, "K" Co., 122" Reg't U.S.C. Troops was born in Shelby Ky, [[strikethrough]]was enlisted the   day of   , 186 , at , by [[/strikethrough]] Drafted, and was mustered in the 17" day of November, 1864, at Lebanon, Ky., by 4" Dist. Ky, for one years. 
At the time of enlistement he was Twenty-Five years old, and Five feet   inches high, Black complexion, Black eyes, Black hair, and by occupation a Farmer. He [[strikethrough]]was discharged (or [[/strikethrough]] died) 20' day of January, 1865, at Louisville Ky
Lt. G S Haseltine was commanding officer, and Albert Cotter first sergeant at time of soldier's death. The following are the names of one or more of his company who enlisted at or about the same time and place: Wilson Parker enlisted 17" day of November 1864, and Fry Bart enlisted 17" day of November, 1864.
