Frank Phelps, Pvt Co, B 114 Reg't U.S.C. Troops was born in Madison Co, Ky, was enlisted the 10 day of June, 1864, at London Ky, by Capt Hays, and was mustered in the 16 day of June, 1864, at London Ky, by Capt Hays, for 3 years. 

At the time of enlistment he was 20 years old, and 6 feet ½ inches high, Black complexion, Black eyes, Black hair, and by occupation a Farmer,. He was discharged ([[strikethrough]] or died [[/strikethrough]]) 2 day of April, 1867, at Brazos Santiago. Tex Lt Conklin was commanding officer, and Robert Jarmon first sergeant at time of soldier's discharge. [[strikethrough]] The following are the names of one or more of his company who enlisted at or about the same time and place: __ enlisted __ day of __ 186 , and __ enlisted __ day of __, 186 .
